CHOICE Review

Describes the development of an integrated social and health services financing and delivery program for elderly people, the Social Health Maintenance Organization (SHMO) that is currently being demonstrated at four pilot sites in the US. The authors first describe problems in the current patchwork of programs that fund services for elderly people and present a conceptual overview of the SHMO and its underlying organizational issues. The text elaborates issues of organizational design for the SHMO, for example, the nature of the relationship between sponsoring agencies and their partners. Detailed descriptions are provided of the systems used directly to deliver services at each demonstration site, as well as a detailed analysis of the benefit packaging, targeting, and medical care policies that affect resource allocation. Marketing and enrollment strategies and the development of financial estimates in the SHMO are discussed and means of managing risk and uncertainty in the development of the SHMO model are assessed. This well-organized volume should be quite helpful to health services administration graduate students, policymakers, and other readers interested in the development of a better system of services delivery for the growing population of elderly people.-L.A Crandall, University of Florida
