CHOICE Review

Long awaited, this is the definitive word on what studies have shown of the effects and probable risks of herbicide exposure in both Vietnam and in industrial and commercial applications. The working committee of the Institute of Medicine has produced a model of meta-analysis with rigorous delineation of the categories of proof contained in presently available research. Indeed, a parallel use of information from industry has elevated what might have been just a rehash of slogans into a deeply probing analysis of the matter. This will not satisfy some with emotional agendas on the issue, but it is good science with a strong call for continuing research, in a text closely edited and heavily annotated. There are just five medical conditions for which present evidence is sufficient to show an association with exposure to herbicides: soft tissue sarcoma, non-Hodgkin's lymphoma, Hodgkin's disease, chloracne, and porphyria cutanea tarda (in genetically susceptible individuals). Several other conditions showed suggestive evidence for an association, all malignancies of some clinical importance.
