| Summary, etc. | La enfermera comunitaria de enlace es una figura gestora de casos que desarrolla su actividad de Enfermería de Práctica Avanzada en el contexto de la Atención Primaria de los Servicios Públicos de Salud. A pesar de que existen modelos similares, como las enfermeras visitadoras inglesas o el dúo médico de familia-enfermera comunitaria de las unidades de atención familiar israelíes, en el modelo español la enfermera comunitaria de enlace se distingue por su alto nivel de autonomía y por ejercer de vínculo entre los distintos niveles de la atención primaria y hospitalaria y los servicios sociales, siempre con un enfoque centrado en la atención al paciente y su familia. |
